Evaluations

We provide court-ordered domestic violence evaluations to determine treatment needs. We use the highest standard of evidence-based assessment tools so that you are never over treated or under treated for your specific needs. 

The evaluation process takes approximately one hour to complete and will be in-person with an interview with a Licensed Clinical Social Worker in which the state-required Inter-Partner Violence & Risk/Needs Evaluation (IPV-RNE) will be completed. If the evaluation is ordered by the court or a probation officer, clients are required to bring a copy of the Police Report from the incident and a recent BCI Report. Completed Evaluations will be sent to the referring agency within 3-5  business days.
 

Evaluation Cost: $100 - Cash or Credit/Debit Card

You must bring a copy of your police report and BCI background report to evaluation.

 

To obtain your police report: Contact the police department or court associated with the city in which the offense occurred, and you can pick up your police report in person or request it via email.

 

To obtain your BCI Criminal Background Report: Go to the Department of Public Safety – Criminal Identification (BCI)​:  3888 West 5400 South - Taylorsville, UT  (Located behind the McDonalds). The cost is $15. 

Contact us today to set up your evaluation. Once your evaluation is complete, we then provide domestic violence treatment through groups or individual therapy.
